Starting physical therapy is an important step toward feeling and moving better. At Optimum Therapies, every patient receives dedicated one-on-one care focused on understanding your symptoms, movement, and goals. Your therapist will work with you to create a personalized, evidence based plan designed to reduce pain, restore movement, and help you get back to the activities that matter most.

Yes. In most cases, you have the right to choose where you receive physical therapy services, even if your physician or surgeon refers you to physical therapy. If you have a referral or prescription, you can typically choose the clinic that’s most convenient for you or best meets your needs.

If you’d like to receive care at Optimum Therapies, our team is happy to help coordinate with your referring provider if needed.

Please bring:

  • Your photo ID
  • Your insurance card (if using insurance)
  • Any requested paperwork
  • Comfortable clothing for movement

Wear comfortable clothing that allows you to move easily. Depending on the area being treated, shorts or a loose-fitting shirt may be recommended.

In many cases, yes. Wisconsin and North Dakota allow many patients to begin physical therapy without a referral, although insurance requirements may vary.

Good Faith Estimate

According to the Good Faith Estimate provision within the No Surprises Act, health care providers and facilities, as of January 1, 2022, must provide a Good Faith Estimate of expected charges upon request. This law pertains to patients who do not have insurance or are not using insurance (private pay).
